Thank you
for selecting Dr. Eliot Ghatan as your dermatologist. Dr. Ghatan
and his office staff will do their best to provide you with
the very best skin care possible.
This brochure has been designed to let you become more acquainted
with Dr. Ghatan. It provides valuable information about the
medical services available at this office as well as some of
our policies and procedures. Please take time to review this
brochure and feel free to ask any questions that you may have.

FEES,
BILLING and INSURANCE
|
There are
basic fees for each visit and for additional treatment procedures.
In order to keep our fees and billing cost to a minimum, payment
is expected at the time treatment is rendered.
Insurance is designed to help you meet the cost of medical service,
but the responsibility for payment is yours. In addition to
Blue Cross/Blue Shield, Medicare, Gill, and Oxford, Dr. Ghatan
participates in a variety of other insurance plans. Please check
with the receptionist to see if your plan is included as a part
of this list.

LABORATORY
|
For your
convenience, if needed, Dr. Ghatan will draw blood in the office
so that a trip to the lab will not be necessary .
When skin biopsies need to be taken, the specimen is sent to
a lab where it is reviewed by a Pathologist and a report about
the specimen is sent to Dr. Ghatan. This also holds true for
blood work and cultures that may be required. You will receive
a bill from the laboratory for their services and you are responsible
for payment directly to the laboratory . These fees may be covered
in part by your insurance plan.

YOUR
FIRST VISIT
|
At your
first visit with Dr. Ghatan, you will be asked to fill out an
information sheet. Please complete it carefully.
The examination on this visit includes a complete skin examination.
This is done for two reasons:
1. The problem your are here for may have other skin manifestations
that you may be unaware of.
2. A complete skin exam may reveal a problem of which you are
unaware, but that requires attention.
If you refuse the complete examination, the doctor will take
care of you anyway, but keep in mind that you may be harming
yourself.

LIPOSUCTION
BODY / CONTOURING
This is the most common method of body sculpturing and is ideal

LASER
SKIN RESURFACING
We are now able to remove surface imperfections and dramatically

of the deeper, healthy skin producing smoother, fresher, young-looking

TREATMENT &. SOFT FORM IMPLANTS
Collagen injection is a safe and effective way of reducing facial
wrinkles and augmenting one's lip line. These procedures may
also be used to reduce the appearance of facial and/or acne
scars.
Facial Implants, which are designed specifically for use in
the deeper frown lines, nasal furrows and midfacial area, come
in a wide variety of shapes and sizes. These implants can be
custom- trimmed by your surgeon for a more natural fit. EPTEE
facial implants are safe, dermal filler designed to recontour
your face and can provide definition to your cheeks and chin
and lips. They can also be used to correct deep facial deformities.
